Pre-K students must be 4 years old by September 1, 2018.
Please bring all the following documents for your child:
- Child's Social Security card and birth certificate;
- Georgia Proof of Immunization Form #3231;
- Georgia Eye/Ear/Dental Certificate Form #3300;
- Proof of Home Address (Current Utility bill or Lease Agreement under parent's name.
- Medicaid or Peachcare Card
